You Have Been Referred For Cardiac Surgery ... What's Next?

You have just undergone a Cardiac Catheterization procedure and you physician has determined that you may benefit from a cardiac surgery procedure. The results of your angiogram have been sent to a cardiovascular specialist who will determine if you are indeed a candidate for this intervention. Once it has been determined that this procedure will be appropriate for you, the multidisciplinary team will begin the process of arranging your transfer to the specialty hospital.

 

Your physician/nurse will inform you about which facility has accepted you for your angioplasty. You will then be given a package that includes information to assist you and your family in making your transfer plans. A social worker from the Cardiology Program will meet you to discuss any concerns you may have about this process.

 

The receiving facility will contact your nurse when a bed is available for you. He/she will then co-ordinate your transfer via Air Ambulance. You will be informed of the estimated departure time as soon as it is known, which is usually late in the evening the day prior to the transfer. You and your escort will be taken to the airport by a land ambulance.

 

As a courtesy, your escort may travel with you on the aircraft, providing that there is space available. As this is not always possible, your escort is encouraged to make other travel arrangements as a back up plan.

 

One small carry-on bag per person will be allowed on the aircraft. It should include a change of comfortable clothing, any personal items (i.e. Toothbrush, comb, deodorant, etc.), along with picture identification.

 

When you arrive in the city of your referral centre, a land ambulance will take you and your escort from the airport directly to the hospital. Your cardiovascular specialist will meet you to discuss the proposed procedure. You can anticipate being in the hospital for 5-7 days after your cardiac surgery.

 

Once your procedure is completed, you are encouraged to ask your nurse about your anticipated time and date for your discharge.

 

When you are discharged from the hospital, you will be responsible to make your own travel arrangements for your return home. You are encouraged to have a credit card available or some alternate form of payment to purchase your flight tickets.

 

Upon your return home, you will have a follow up appointment with your physician. You are encouraged to discuss a referral to the Cardiac Rehabilitation program at this time. A member of the program will contact you to discuss your participation when they have received the referral from your physician.

 

Northern Health Travel Grant

 

Northern Health Travel Grants, funded by the Ministry of Health and Long Term Care Of Ontario, are available to residents of Northern Ontario who must travel elsewhere in the province to receive care. This grant will help you to pay for some of the costs associated with your return trip home.
The Cardiology program social worker, or any member of our health care team, can assist you with this application process. We will provide you with the appropriate form when it is confirmed that you will be transferred to another centre.

 

Northern Travel Grant Instructions
Section 1
To be completed by patient or health care team member, which provides personal information about you.
Section 2
This section is only completed when an additional agency is required to assist in your transfer process.
Section 3
Your referring physician will complete this section of the grant form. This will preferably be done before your transfer.
Section 4
At the referral site, the specialist will complete Section 4.
Section 5
If the physician feels that a companion is required during your transfer and indicates such under Section 3, Section 5 must be completed by the person who travelled home with you.

 

Upon return home, you will be required to submit the Travel Grant application accompanied by original tickets/stubs/receipts. It usually takes 8-10 weeks for your application to be processed and reimbursement of funds to take place.
