In
an effort to ensure long term success, it is essential that
those referred for bariatric surgery are appropriately assessed
for eligibility, that mental health/behavioural issues are
identified pre-operatively and that short and long term treatment
plans are established. A clinical team of specialized and
dedicated health professionals, using an interprofessional
collaborative model of care, will support patients to achieve
sustained lifestyle changes toward mental health, nutritious
eating and regular physical activity.
The health care team at the Thunder Bay Regional Bariatric
Care Centre consists of an Endocrinologist, Nurse Practitioner,
Psychologist, Social Worker, Kinesiologist, two Registered
Dietitians, a Program Coordinator, along with Clerical Support
and a Patient/Family Advisor. We are committed to providing
Patient and Family Centred Care, high quality pre surgical
assessment and post surgical follow-up services for adults
that meet established criterion for bariatric surgery.
Clerk/Receptionist
Books patients into education classes and schedules appointments
with members of the healthcare team.
Physician
Examines and reviews patient information and test results
before and after surgery.
Makes referrals to Bariatric Surgeon and other healthcare
specialists.
Nurse Practitioner
Assesses and maximizes patient’s health status throughout
treatment.
Coordinates with Physician to provide and implement individualized
plan of care.
Provides group and individual education.
Registered Dietitian
Assesses patient’s ability to change eating habits.
Provides guidance and support while patients adapt to new
eating routines.
Psychologist/Social Worker
Assesses patient’s ability and motivation to conform
to significant lifestyle changes.
Provides individual guidance and counselling.
Kinesiologist
Assesses individual level of physical activity.
Provides strategies to attain optimum levels of functioning.
